The role of dipeptidyl peptidase-4 inhibition in correcting disorders of fat metabolism in patients with type 2 diabetes mellitus and obesity
Abstract
Type 2 diabetes mellitus and obesity are closely interrelated metabolic disorders characterized by insulin resistance, β-cell dysfunction, dyslipidemia, and abnormal adipokine secretion. The present study evaluated the metabolic effects of adding the DPP-4 inhibitor gozogliptin to metformin therapy in patients with type 2 diabetes mellitus and adipose tissue dysfunction. Sixty-four patients were included and divided into two groups: 40 patients received gozogliptin plus metformin, and 24 patients received metformin monotherapy. Clinical, laboratory, and bioimpedance parameters were assessed at baseline, after 12 weeks, and after 24 weeks of therapy. The combination therapy resulted in a greater improvement in glycemic control, with a significant reduction in HbA1c and fasting plasma glucose, along with a decrease in insulin resistance indices and an increase in β-cell functional reserve. In addition, the combined treatment produced more pronounced favorable changes in body composition, including a reduction in fat mass with preservation of active cell and skeletal muscle mass. The therapy was also associated with a beneficial shift in adipokine profile, characterized by increased adiponectin and decreased leptin levels, as well as improvement in lipid parameters.
Biography
He was born on 26 June 1996. She is currently a PhD student at the Department of Endocrinology, Russian Medical Academy of Continuous Professional Education, Ministry of Health of the Russian Federation, and she has successfully completed her pre-defense and is now preparing for her dissertation defense. She works as an endocrinologist in Moscow, providing clinical care for patients with endocrine and metabolic disorders. Her main scientific interests include type 2 diabetes mellitus, obesity, metabolic syndrome, adipokines, and modern approaches to metabolic correction. She is actively engaged in research, publishes scientific papers, and presents her work at national and international conferences. As a young clinician-scientist, she combines practical medical experience with academic investigation and aims to contribute to the development of evidence-based endocrinology and improve patient outcomes. These findings suggest that DPP-4 inhibition combined with metformin has a positive effect not only on carbohydrate metabolism, but also on fat metabolism and body composition in patients with type 2 diabetes mellitus and obesity. Such an approach may improve metabolic health and support more effective long-term diabetes management.
